Thank you for considering us to be your oral health care provider. Our goal is to make your dental visit a good experience. For many people, going to the dentist can be stressful. We want your experience at Easy Dental to be stress free and healthy.
How to prepare for the first visit:
- Bring completed new patient forms, driver’s license and dental insurance card
- You may bring radiographs that are less than 3 months old. Radiographs have to be digital and easy to read. If these requirements are not met, then we will take new ones at the first visit.
- We do a plaque sample analysis and this means that we ask that you do not brush and floss for 24 hours prior to your appointment.
- A 1 hour long appointment with doctor which includes oral exam, discussion of exam and plaque findings, biocompatibility testing if necessary.
Our goal is to maximize your out-of-network dental benefits and minimize out-of-pocket expenses. As a courtesy, we will file your dental claim form to your insurance company to assist you in getting reimbursement, and provide a receipt for the services rendered. Payment is due in full at the time of service.
We accept cash, check, MasterCard, Visa, Discover.
We also offer flexible payment options with Care Credit financing, a healthcare credit card company that allows you to defer your out-of-pocket expenses without interest.
WE DO NOT OFFER IN HOUSE FINANCING OR PAYMENT PLANS.
As a courtesy we will file dental insurance claims on your behalf, but designation is made for insurance payments to be sent directly to the patient or subscriber. We do not accept assignment of benefits from insurance, we are not in-network provider for any insurance company, nor do we participate in discount plans of any kind.
APPOINTMENT CONFIRMATION POLICY
We will contact you several days in advance to confirm your appointment. If you do not respond to the first confirmation request, we will make a second attempt to reach you using all contact methods provided by you. If you do not respond to the second confirmation request, either by phone(leave message), by text or email, within 3 days prior to your scheduled appointment, the time will be given to another patient who is waiting for an appointment.
Three business days advance notice is required to appointment reschedules and cancellations. A broken appointment fee will be charged if adequate notice is not provided, except in cases of genuine emergency.
